Membership Includes:
Annual Comprehensive Physical Exam
Preventative Care including nutritional counseling, weight loss, and smoking cessation
Management of chronic conditions and medications
Acute Care for illness and injuries (excluding Emergency Care)
Virtual Visits by email, text and phone
In-office tests including EKG, Urinalysis, Rapid Strep tests
Amazing Lab Discounts
Minor procedures (excluding charges for pathology)
Membership
There is a one-time non-refundable application fee for new enrollment of $85.

Monthly Membership Fees
Children 13-19 years old, $30/month with at least one parent membership (Otherwise $55/mo)
Adults 20-44 years old, $55/month
Adults 45-64 years old, $70/month
Adults 65+ years old, $85/month
Employer groups with 5+ employees, $55/mo/adult
