Nexus Letter Services for Veterans

What is a Nexus Letter

At Living Well Family Medicine, we are honored to support Veterans by providing independent medical Nexus letters when clinically appropriate.

This service is available to Veterans whether or not you are a current patient of the practice. It is offered as an additional professional service separate from our membership-based primary care.

A Nexus letter is a detailed medical opinion that connects a Veteran’s current medical condition to their military service. These letters are often used as part of a VA disability claim.

A Nexus letter must include:

  • Review of relevant medical records

  • Review of military service records (if applicable)

  • A medical opinion supported by clinical rationale

  • Documentation stating whether the condition is “at least as likely as not” related to service

Who Is Eligible?

Non-Patients

Veterans do not need to be established patients of the practice to request this service.

Established Patients (Membership Benefit)

Active members of Living Well Family Medicine who have been established with the practice for a minimum of 3 months with documented ongoing care may qualify for a reduced rate.

Living Well Family Medicine does not establish new patient relationships solely for the purpose of writing a Nexus letter.

Our Process

    Record Submission Required

    All relevant medical records and service documentation must be provided prior to scheduling. We do not obtain records on your behalf.

    Virtual Evaluation

    The visit will be conducted virtually.

    Comprehensive Record Review

    We conduct an independent and objective review of your documentation.

    Medical Opinion

    If medically supported, a Nexus letter will be completed. If the medical evidence does not support a nexus, this will be discussed with you.

Fees

$1,200 – Nexus letter for one medical condition
(Includes record review and up to 2 hours of professional time)

  • Additional Time
    If total review and preparation exceeds 2 hours, additional time will be billed at:

    $400 per hour, billed in 30-minute increments.

Payment is required prior to release of the letter.

  • We do not guarantee any specific disability rating or service connection percentage.

  • Payment is for time, expertise, and professional medical opinion — not for a guaranteed outcome.

  • Submission of records does not guarantee that a Nexus letter supporting service connection will be written.

  • This service is not covered by insurance.
